The Human Diver: Human Factors in Diving recently filmed a documentary about a tragic diving accident that occurred in May 2018. Diver Brian Bugge entered the water while breathing from his CCR. Unfortunately, his pre-dive checks were incomplete and he died because he went hypoxic on or near the surface and drowned. It would have been easy to put the root cause down to Brian not doing his pre-dive checks, but things were a little more complicated than that.

Brian's widow Ashley and the rest of the dive team came together in November 2019 in Hawaii to film a documentary about that fateful day and the events leading up to it. This is a documentary shot through the lens of human factors and Just Culture, produced in a way to help others learn and prevent such events from occurring in the future.
